Mr. Adams takes a visionary approach to the problem of accident reduction by proposing management organizational changes which, if implemented successfully, will lead to the reduction of occupational hazards. Adams recognizes that the changes call for fundamental alterations in traditional management and supervisory reporting systems, and views these changes through the perspective of TQM principles, resulting in the elimination of all types of wasted resources at all levels.

Adams' breakthrough is to apply TQM principles to accident causation theories based on the accidental release of energy. In doing so, he found that the risk analysis tool developed by W.G. Johnson, the Management Oversight Risk Tree (MORT), started with the same fundamental assumption: management planning or operational oversights are the source of 85% or more of the system variations resulting in accidents.

Adams provides a detailed step-by-step analysis of how the MORT can be used to effectively create a bridge between management at all levels and the general work force, leading to a system-wide reduction in hazards.
